Advance Directive
An advance directive is a legal document that allows you to spell out your decisions about end-of-life care ahead of time. It is used to guide your loved ones and health care team in making clear decisions about your health care if you can’t make medical decisions by yourself. An Advance Directive helps ensure that your wished are carried out, even when you are unable to communicate them.
