Sports Physical Week
July 31st – August 4th, 2023
Family Medicine at Hayward Area Memorial Hospital will be offering sports physicals for student-athletes from any area school district the week of July 31–August 4, 2023. Athletes can receive a full physical, a sports physical, or assistance in completing alternate-year cards. Providers Katrina Gardner, MD, Karlye Wisdom-Rodriguez, MD and Lisa Bratvold, NP will be providing care.
Wisconsin Interscholastic Athletic Association (WIAA) requires a sports physical, valid for the following two school years. In the year when an examination is not required, an Alternate Year Athletic Permit Card must be signed by a parent or guardian and be on file. Athletes must have their physicals completed before the first day of practice for their sport.
During a sports physical, a medical provider will assess an athlete’s past medical and social history. They will also go through basic height and weight, and check for any concerns or abnormalities. Heart and lung health is checked to determine an athlete’s ability to participate in physical activity without putting themselves at risk for cardiovascular concerns. Shortness of breath or a lung condition can often go unnoticed, but a medical professional can address these problems.

Students under the age of 18 must be accompanied by a parent or guardian. Please bring a valid photo id and current health insurance information.
